GMT</pubDate></item><item><title><![CDATA[傷口換藥注意事項]]></title><link>https://tpech.gov.taipei/mp109171/News_Content.aspx?n=FE7461839666A504&amp;s=55B6B1FC1E3383D5</link><description><![CDATA[<div>一、目的：出院後，病人在家中能自行傷口換藥。二、注意事項：1.保持傷口乾燥及周圍皮膚清潔（以擦澡代替洗澡），預防傷口感染。2.每日觀察傷口變化，是否有紅、腫、熱、痛，異味或分泌物。3.癒合中的傷口容易發癢，切勿用指甲搔抓，以指腹按摩傷口周圍為宜。4.拆線視傷口部位及醫師評估後決定，一般來說頭部約7天；軀幹約7-10天；手、腳約10-14天。5.拆完線後傷口以美容膠或紙膠黏貼，隔日即可沖水洗澡。三、用物準備：消毒棉棒、紙膠、優碘藥水、消毒紗布、無菌生理食鹽水。四、消毒方式：1.換藥前後須洗手，保持手部清潔。2.先使用無菌生理食鹽水棉枝消毒，由傷口中心點向外圍環形擦拭，請勿來回反覆擦拭。3.再使用優碘藥水同樣從傷口中心點向外圍環形擦拭消毒，並停留30秒。4.重複使用無菌生理食鹽水棉枝消毒，將優碘藥水去除。5.換藥中勿對著傷口咳嗽或說話。6.蓋上紗布，紙膠黏貼。若分泌物多或紗布髒時，隨時更換。7.用物收拾妥當，以免灰塵掉入造成傷口感染。</div>]]></description><pubDate>Thu, GMT</pubDate></item><item><title><![CDATA[認識乳癌手術]]></title><link>https://tpech.gov.taipei/mp109171/News_Content.aspx?n=FE7461839666A504&amp;s=A3E78BD2FA7D1D8A</link><description><![CDATA[<div>一、手術目的：主要目的是將乳房及腋下的淋巴腺切除。手術方式可分為：1.乳房保留術：將癌瘤其周圍的乳房組織及腋下淋巴腺摘除，保留大部分乳房。適用於早期乳癌或小型乳癌，但術後必須做局部放射線治療，為目前最常用術式。2.改良式根治性乳房切除術：全部乳房組織切除但保留胸大肌，可預防胸部凹陷，減少上肢淋巴水腫及肩膀活動功能受限。3.根治性乳房切除術：切除全部乳房組織及腋下淋巴結，保留胸小肌。適用於癌細胞侵犯胸大肌及淋巴結轉移，術後易引起上肢淋巴水腫與活動受限。二、手術前準備：1.完成手術及麻醉同意書。2.皮膚準備及身體清潔：手術前一天，需全身沐浴、更衣及洗髮，皮膚準備原則：(1)以沐浴乳清洗即可。(2)勿用力搔抓皮膚，造成皮膚受損。(3)若手術部位有傷口，清洗手術部位上下段即可。3.手術前禁食：至少6-8小時，禁食的內容包括任何藥物、食物、水及飲料等。三、手術後注意事項：1.活動方式：當麻醉清醒後，可由健側手臂支撐坐起，通常手術當天即可下床活動，活動時間視個人耐受狀況逐漸增加。早期下床活動有助於身體功能恢復。2.傷口照顧：(1)手術後患側的傷口以棉墊覆蓋及繃帶包紮，若您感到太緊而致胸悶不適或傷口有滲濕出血情形，請告知護理人員。(2)手術後，請保持傷口清潔乾燥，醫師與專科護理師會視傷口狀況協助您換藥。</div>]]></description><pubDate>Thu, lang="en-us"><o:p></o:p></span></p><p class="style171"><span lang="en-us">(b).</span>而接受乳房保留手術<span lang="en-us">+</span>前哨淋巴切除術者，術後<span lang="en-us">1</span>～<span lang="en-us">2</span>天一般會以散紗壓迫傷口，以減少傷口出血、血腫產生。 <span lang="en-us"><o:p></o:p></span></p><p class="style171"><span lang="en-us">2. </span>出院後，大部分乳房和（或）腋下的傷口不須換藥，請保持傷口清潔乾燥。<span lang="en-us"><o:p></o:p></span></p><p><span class="style1711"><span lang="en-us">3. </span>出院後，病患傷口如置放引流球<span lang="en-us">(Vaccum ball)</span>引流傷口血水及分泌物，引流管插入口傷口應每日換藥護理，引流球需維持壓扁狀<span lang="en-us">(</span>負壓引流<span lang="en-us">)</span>，引流管呈拋物線狀以紙膠布井字型固定，應避免拉扯、壓折和堵塞，並記錄引流量回診時供醫師參考，一般在每日引流量＜<span lang="en-us">20</span>～<span lang="en-us">30cc</span>時，醫師會視情況而予以拔除。 </span><span lang="en-us"><o:p></o:p></span></p><p><span class="style1711"><span lang="en-us">4. </span>個人衛生方面，如：洗澡。若有引流管，在腰部以上可用擦澡，而腰部以下可以用沖澡的方式。而在引流管移除後的隔天，在胸部的傷口用防水敷料覆蓋保護下，則全身可以淋浴。 </span><span lang="en-us"><o:p></o:p></span></p><p><span class="style1711"><span lang="en-us">5. </span>注意傷口及引流管如有紅、腫、熱、痛或異常分泌物、發燒情形，請立即回診。 </span><span lang="en-us"><o:p></o:p></span></p><p><span </span>復健方面：手術後術後復健運動，可減少手臂淋巴水腫，並恢復肢體功能。經醫師指示後可在術後開始做復健運動，最初每日<span lang="en-us">2-3</span>次，每次二十分鐘，循序漸進增加次數及份量，以不感覺疼痛為主。以後每天做下列運動及日常生活動作如梳頭、洗臉、穿衣等，使手臂功能恢復正常。（詳細的復健運動方式可參考『乳癌的術後復健運動』的說明） </span><span lang="en-us">)</span>，引流管呈拋物線狀以紙膠布井字型固定，應避免拉扯、壓折和堵塞，並記錄引流量回診時供醫師參考，一般在每日引流量＜<span lang="en-us">20</span>～<span lang="en-us">30cc</span>時，醫師會視情況而予以拔除</span></span>。<span lang="en-us"><o:p></o:p></span></p><p><span lang="en-us">2. </span>出院後，大部分傷口保持乾淨即可不需換藥，並按醫囑<span lang="en-us">(</span>約手術後一週<span lang="en-us">)</span>，至一般外科門診追蹤。<span lang="en-us"><o:p></o:p></span></p><p><span lang="en-us">3. </span>部分病患手術後有輕微之吞嚥障礙，此症狀會逐漸改善。<span lang="en-us"><o:p></o:p></span></p><p><span lang="en-us">4. </span>手術後前頸部有些許浮腫或壓迫感為正常現象，短期內會消失。<span
